This questionnaire assists in the identification of neuropathic pain. It does not require a doctor to assess it.

There are some questions regarding it's efficacy in FM and central sensitization disorders though

The PainDETECT questionnaire is another questionnaire which has been well validated in studies, and has an advantage in that it is solely based on answers that a patient can give for him/herself, with no requirement to be examined at the time of the questionnaire.
