Social Work/case Management?

My clinic G.P. doctor gave me a referral to the clinic's social work case manager, but someone closed the request without me ever getting to speak to anyone. My doctor told me to ask the office manager about it, so I did. She said that they still offer the service and that she didn't know who closed it out, but that she would check on it for me. She acted as if she would, but then her look changed and she just dropped the matter. Can anyone please tell me what a social work case manager would do for someone?
